The posting, in Clover Health's own words
archived Oct 21, 2026At Clover Health, we are committed to providing high-quality, affordable, and easy-to-understand healthcare plans for America’s seniors. We prioritize preventive care while leveraging data and technology through the Clover Assistant, a powerful tool that helps physicians make informed health recommendations. By giving doctors a holistic view of each member’s complete health history, we ensure better care at a lower cost—delivering the highest value to those who need it most. Our Clinical Wound Care & Home Health team combines technology and clinical judgment to provide coordinated, whole-person care that helps members heal and remain safely at home. As a Wound Care Certified Nurse Practitioner, you will oversee care for members with complex and chronic wounds from initial assessment through closure or transition of care. You will review high-definition wound images and clinical data through a secure, AI-enabled platform, independently apply clinical judgment, and address broader health and social needs affecting healing. As Clover expands its Home Health program, you will also support clinical review, care coordination, and program development.
As a Wound Care Certified Nurse Practitioner, you will:
Manage wound care through closure. Develop individualized, evidence-based treatment plans, including dressings, monitoring frequency, and home-health interventions within scope of practice; reassess progress and determine when to close, extend, or revise care. Assess wounds using clinical data and technology. Review images, measurements, history, and healing trends, using AI-supported tools to inform assessments while independently determining diagnosis, treatment, and escalation. Recognize complications and escalate promptly. Identify stalled, worsening, or complicated wounds and coordinate timely evaluation by physicians, surgeons, podiatry, vascular services, infectious disease, or emergency care as indicated. Address the whole member. Evaluate mobility, pressure relief, nutrition, diabetes, vascular health, infection risk, medications, functional status, caregiver support, and social needs affecting healing or overall health. Maintain accurate documentation and diagnoses. Document assessments, clinical reasoning, treatment changes, escalation decisions, and outcomes promptly; ensure relevant diagnoses and coding are supported by clinical evidence and applicable standards. Coordinate care across providers. Partner with home-health clinicians, primary care, specialists, care managers, pharmacists, and social workers to review updates, respond to changes, and maintain a cohesive care plan. Support Home Health clinical oversight. Review clinical needs, appropriateness of services, progress against goals, and opportunities to improve coordination as the program evolves. Help build the program. Collaborate with clinical, operational, product, and technology teams to improve workflows, protocols, escalation criteria, documentation standards, and quality measures.
